Bring a current list of prescription medications, over-the-counter medications, and supplements, along with your preferred pharmacy information.

Be prepared to discuss previous diagnoses, treatment, hospitalizations, medication trials, relevant medical conditions, and substance use history.

Have relevant records or recent laboratory results available when applicable, and consider writing down the symptoms, concerns, questions, and treatment goals you would like to discuss.

Cancellations & No-Shows

Please plan to be available at your scheduled appointment time. For telehealth visits, choose a private location with a reliable internet connection where you can speak comfortably and confidentially.

If you need to cancel or reschedule, please provide as much advance notice as possible. Cancellation and no-show policies, including any applicable fees, will be provided during the registration process.

Medication recommendations are based on an individualized psychiatric evaluation and clinical need. Prescriptions, medication changes, and refills are provided when clinically appropriate and are not guaranteed solely by scheduling an appointment.
